The order form is a short questionnaire and the information provided is used to create your child's letter. There are 3 ways to submit your order: Option 1 / Open it now and print it out to be mailed by post...: You can open the PDF Order Form by clicking here. Then print out the form, complete it and mail it back to us at the Jodee Spalding Option 2 / Receive it as an email attachment...: To have a copy of our Order Form sent to your email address please send a request to jodee [@] jodeewrites.com. When you receive your order form open it, complete it, save it, attach it to an email and send it back to us. Send your email with your order form attached to jodee [@] jodeewrites.com. Option 3 / Download it now...: To download and save our order form to your computer right now, click here. Adobe Acrobat will open. Click "File" and then "Save". Choose a location to save the file and click the "Save" button. When the download completes you can open it, complete it, save it, attach it to an email and it back to us. Send your email to jodee [@] jodeewrites.com. Payment When we receive your completed form you'll be forwarded a PayPal invoice by email. Pay the invoice to complete the order.
